The rate applies to the check-up regarding all the medication you specify and not per medication.

Pharmacist Drs DH Siem will check the combination of your regular and alternative medication for you for safety. You will receive your recommendation by email ASAP.

You will receive a questionnaire from Blood Values Test:

• Do you use regular medications and if so, which ones:
• Do you already use nutritional supplements and if so which ones:
• For what complaints do you want to use nutritional supplementation?

Which you mail to the pharmacist.

If an interaction advice is very complex and time-consuming, we will charge a double advice (50 eu) This will be communicated to you by the pharmacist in advance based on the completed questionnaire and preliminary examination.
